The group medical, dental and vision plans renew February 1st; all enrollment and plan changes will be effective on that date.
EMPLOYEE CONTRIBUTIONS:

- After 90 days: 50% of Blue Shield Silver PPO Plan for employee only
- After 3 years: 75% of Blue Shield Gold PPO Plan for employee only
- After 5 years: 75% of Blue Shield Gold PPO Plan for employee, 50% toward dependent costs of the same plan
- Dental and Vision plans are voluntary and employee paid
WAITING PERIOD
Employee coverage will begin on the 1st day of the month following 60 days of employment. An example:

- Date hired February 1st.
- 60 days employment would be April 1st.
- Coverage start date on May 1st.

Annual open enrollment is in the month of January. For new employees, open enrollment is during your 90 day waiting period.
