After the annual open enrollment, your benefits will start on October 1st. For new
employees, your benefits will start on the 1st of the month following 60 days of full-time employment. Your employer pays 100% of your employee premium for the Anthem Blue Cross Gold PPO plan. Dependent costs will be withheld from your paycheck. See dependent rates in chart on the back. Self-refer to any Primary care doctor or Specialist in Anthems Nationwide PPO Network. Download Anthem’s Sydney Health app for 24/7 access to your benefits and claims information, ID cards, virtual doctor visits, and more. FLEXIBLE SPENDING ACCOUNTS:

Flexibe spending accounts (FSA) are now available and will help supplement your benefit package. You can use your FSA funds to pay for eligible medical, dental and vision expenses. The FSA is employee funded. Flexibe spending accounts are 12 month interest and tax free plans that help you save money.
YOUR BENEFITS CARD:
Employees who participate in the new HRA and FSA plans have easy access to their funds to your Summit debit card. These cards will arrive in the mail around your plan effective date.

Studio 4 Forty’s dental and vision plan can be used at the provider of your choice. Dental and vision plan costs are covered by the employee. Participation is voluntary.
